Prior to your initial visit, we recommend you call the Member Services Department (contact information is on the back of your insurance card) to verify eligibility and benefits for out patient speech therapy services.

Each individual plan is different and services may have a deductible, co-insurance or co-payment due at the time of services.

Please note that although we are contracted providers with the above companies, participation does not guarantee coverage of service. Some plans do not cover speech-language therapy services unless they are "medically necessary".  Your insurance company can explain any restrictions/exclusions that may apply to your individual plan.

CSHS does also offer private pay rates in the event that your plan does not cover the needed services or if your plan is not one of the above listed plans. Please inquire directly with CSHS for fee schedules.

All payments are due at the time of services.
